Grant Proposal
If you would like to apply, your concise written proposal must include:
Complete information about your organization, including its history, purpose and operations;
Your organization’s current operating budget;
The project’s budget;
Information about your governing board, including a list of members and their professional/business affiliations;
Audited financial statements;
A copy of your organization’s IRS determination letter;
You should describe the specific use for the funds requested as well as a brief description of how you intend to evaluate, at the conclusion of the grant cycle, the success of the program or project the requested funds would support. Your proposal should be signed by the CEO of your organization and your organization’s Board Chair and/or include a letter of support from the Board Chair.
